Not paying claim for surgery

Submitted a state with Mixed Insurance for my hip replacement surgery. I had been identified in the era of 4 with Juvenile Rheumatoid Arthritis Symptoms. I'd my first joint changed in 2006. They settled for my surgery period in addition to the full time I had been impaired for. I subsequently had both my legs changed in 2007 plus they again settled the state like these were suppose to. I'm today 23 and also have had my other hip changed. They're currently declining to pay for my state. They've lied in my experience on every event, have explained they have delivered words to my physicians to find out more subsequently i discover they never delivered something, etc... All I'd like is my state to become managed pretty and properly. They're not informing me that since I have WOn't get over Arthritis they cant spend my state. Easily recall precisely, my state was for my hip-replacement surgery not for having arthritis!? I will and certainly will get over my hip being changed and really should be taken care of the full time I'm handicapped. I've settled into this plan since I have was 4 yrs old which may be the treament i get? I truly wish to and have to document case against them in addition to a using the DOI.
