Professional charges for services not sought, required or received

My name is MARY-J. CASTELLANO, 76 yrs old, outdated, fragile and struggling with numerous medical issues and physical restrictions. I create the next Declaration under charge of perjury.

On February 14 I visited Gem Dental Treatment and Cypress Dental, Dr. Ismail Elsherifis workplace, and requested the dentist to repair just one free limit and re-concrete and secure it in location — nothing else. The dentist and his team proceeded to provide me x rays and consider thoughts, about that we really questioned but couldn't perform a factor when I lay about the dental chair, convinced that all of the methods were possibly essential for the Dr. Elsherif to repair my one free limit. Used to do not understand. Used to do not visit the dentist frequently. Later I discovered that Dr. Elsherifis office billed my charge card more than 1,100.00

Uncertain that I truly required all of the x rays, thoughts, etc. For my only-one-free-limit issue, I visited Individual Dental Service/Lidia A. Mendoza, D.D.S., General Dentistry, at 974 North Mountain Method, Ontario, California 91762, on June 9, worrying just of and requiring my one free cover guaranteed. Without much ado, Dr. Mendoza merely re-cemented and guaranteed it in position and charged me-only $160.00 on her services. I've not had exactly the same issue since.

My complaints against dr. Ismail elsherif and his office:

Therefore among my grievances against Dr. Ismail Elsherif and his office are: (1) They billed me $1,100, atleast, for "providers" not wanted, needed or obtained (particularly, for example, Dr. Elsherifis proposed notice to my charge card business dated May 27 statements I had been "anesthesized" that we emphatically refuse); (2) Dr. Elsherif didn't first show me the methods, and requirement for them, or did he first acquire my complete, informed agreement to whichever procedures he promises he did; and (3) Providers that might have now been made and procedures completed were NEITHER REQUIRED OR ANTICIPATED and, today I've determined, NEEDLESS for the only real cause I contacted him for: to possess my one free limit re-cemented and guaranteed in position.

Our efforts to challenge what I contemplate as Dr. Elsherifis improper and incorrect costs and cost produced by my charge card organization to him have now been to no avail. I'll follow my initiatives within this respect and find full refund.
