Misbilling, over charges, late charges applied when paymants were made to agreement

On aprox 7/15/08 I received dental work and signed an agreement to pay monthly payments of $100 each. I received work on two teeth in which was completed the work that was completed was two bulid up pins and two canals, they billed my insurance on 7/17/08 for four canals, and two build up pins.

They over billed the insurance company for 2254.00 it should have been 1656.00 the insurance company adjusted the billing and payed 1100.00 leaving me with a payment of 556.00. I had also had a exam and xrays they billed the insurance 113.00 but it should have been 87.00 leaving me with the 87.00.

I had agreed to 100.00 a month payment and some where they adjusted my payment to 120.00, they never contacted me concerning this matter, I have clled concerning this matter and have brought it up to them when they call but it has never been taken care of and I receive late payment call and charges because of it.

On 7/19 they rebilled my insurance company for another biuld up pin but I only received two this would make number three. Again on 7/19 on a seperate bill they billed the insurance company another build up pin this would make four I only received two.

On 8/2 they billed my insuranc company four 3 crowns I received one crown covering two teeth and a hole.

On 10/28 I received a bill in the mail stating that my insurance denied claim, but according to the agreement they only expected 1100.00 and they were now increasin my payment amount but never said haow much and now said I was behind by 80.00 and they were charging me a late fee.

I have asked for a billing showing adjsutments and received one showing 3 build up pins (I only received 2), 2 root canals (correct), 3 crowns (received 2).
