Prescription filled 2X by CVS in one month - no safety limits prevented this - Prescription Mail Order Service

In November I visited my Dr. Who wrote me a paper prescription for a 90 day supply of a maintenance med I filled it on 12/17 at my local CVS pharmacy. On 12/18 CVS sent me mail order for a 90 day supply of the same med. I didn't open the package, refusing it back to the post office.in Feb. 2013 I received a bill from CVS Caremark for the mail order prescription. I called CVS. They said it was phone ordered by my Dr's office (who has never sent this to them before). I do not use CVS Caremark mail order for maintenance medicines. They said they have no record of receiving the package returned so they won't clear the bill although I did not order it and it should not have been filled at either the mail order or the pharmacy. I made a dispute with CVS Caremark - they told me I could call to check on it. A month later their collection dept. Called me requesting to pay it. This person said CVS Caremark determined I owe it since they don't have record of receiving it back. This is not surprising because their dispute is with themselves, not a 3rd party. They threatened to send the bill to collections. I cannot complain to my prescription insurance company because it is also CVS Caremark. I reluctantly paid this $20 bill but feel I have been bullied and they should take responsibility to not double fill prescriptions. This is unsafe that a company did not have a system to prevent the same prescription med from being filled two times within a week without calling the patient.
