Charge my insurance and bill me for service that is way over and not getting those renal services that I am being charged for this has been going on for approx. 6months

I have been receiving renal treatment from davita for approx. 6 months. During the months I have received treatment. I am normally dialyzed 3hrs to 3.5 hrs. I am being over charged greatly every month. They are charging me for four hrs. Sometimes five and overcharging my insurance company for services not receiving. Please help me resolve my medical issues with this company thanks.
