Mail-in rebate for a free game never arrived after 8 weeks

I sent in a rebate for a free game on 09/01/04. It said that the rebate will be to my house in 8 weeks. It is still not here.

I tried to e-mail them, but they did not respond. They do not have any other way to contact them that I have found.
