USBI, Norristown Telephone Unauthorized Charge by USBI on behalf of Norristown Telephone, They're in PA, I'm in OR Blue Bell

When I received my November Qwest Phone Bill, I noticed a charge that I didn't recognize and did not authorize. The charge was from USBI on behalf of NORRISTOWN TELEPHONE. I called Qwest inquiring about the charge and they told me that it was a 3rd party company billing. They gave me a credit of the $15.43 and put a block on my account to block any 3rd party billing, they advised me to contact USBI and NORRISTOWN TELEPHONE. I called USBI they forwarded my call to NORRISTOWN TELEPHONE. When I spoke to Norristown Telephone they tried to tell me that that charges were legal and legit. I advised they that I am the only one on the phone bill and that I had not made or authorized any changes. They wanted to know if I knew a friend's e-mail address and then gave me her name. They claimed that they had received an e-mail from her. I advised them that she did not have authorization to make any changes to my phone. They then claimed again that the charges were legal and legit. I then told them that they had about 3 seconds to cancel the account or the next call would be to F.C.C. And to Oregon P.U.C. To discuss slamming. They then claimed to cancel the account, I requested a copy of the e-mail including all headers and was told that they couldn't provide me with it. I asked what day was the account created and was also told that they couldn't provide me with that info. They told me that I had to contact the corporate office for that info, I requested a phone number for the office and was refused, was told that the only way for me to contact them was by e-mail or postal mail box.
