Shipped using ex-husband's name-I NEVER mentioned him-not lived at address in 14 yrs

I placed a phone order and used my name and address. It was billed to the credit card I gave, which is in my name only. When the item arrived, the name on the the shipping label was my ex-husband's name at my address. He has not lived at the address for 14 years and has nothing to do with that credit card.

I want to contact the company and see how that name is even assocaited with my order as I NEVER mentioned his name in any part of the order. I am concerned that he is somehow linked to my account.

As there is no packing order or phone number, I have no way of contacting this company, but want to protect myself.
