My name is Dominick and I have been a victim of medical malpractice. Ive had to have four surgeries by 3 different doctors on my neck and I am in pain every day. I had fallen at work back in went to the company clinic, complained to the Dr. About numbness in my right leg and right arm and pain in my back and my neck. Dr. Took x-rays of my back and informed me nothing was wrong. I had fallen on the cement floor flat on the right side of my back. I continued to have pain in my back & neck and numbness in my right arm and right leg, but I kept working, as the doctor said there was nothing wrong. I fell again at work in head first into a cement wall, resulting in these aforementioned surgeries on my neck.

I hired an attorney, Michael Higgins, informing him of the 2008 injury and I wanted to file a claim. He informed me he did file a claim. I ended up firing Mr. Higgins and hired another attorney, Dina Ninfo, in June. This is when I found out that Mr. Higgins did not file any claims from only 2009.

I went to my family Dr., Dr. Denis OConnell, complaining about the pain in my back. Dr. OConnell sent me for an MRI, which was submitted under my wifes insurance, since Liberty Mutual would not agree to pay. Dr. OConnell did not see what could be causing my pain, so he referred me to Dr. Drake. Dr. Drake took x-rays of my back and also my hips. My hips are where he saw a problem and so he referred me to his colleague, Dr. Kuesis. Dr. Kuesis ordered an MRA, which showed I have a piece of bone floating around in my hip, which was from an old fracture.

Liberty Mutual refuses to authorize surgery for my hip, as there is no proof that I injured my hip when I fell at work in as stated above. I informed my new attorney, Ms. Ninfo, that I wanted to file the 2008 claim, but she refused to file. As you will read in my document, I highlighted all the points of interest. Im in pain every day. I just cant take it anymore. Dr. OConnell also had referred me to the Mayo Clinic in Minnesota and they accepted me. I informed Ms. Ninfo that I was accepted to the Mayo Clinic. Her statement to me was, oh, Ill have to update them on your file. A week later I received a letter from the
Mayo Clinic that I was rejected. I contacted the Mayo Clinic again today, July 2 to see if they would accept me for my hip surgery. I asked them if the bone floating in my hip could be analyzed to find out if it could have been caused during my fall in 2008. She did not know, but said I would have to ask the doctor.

I plea for a Federal investigation into why the attorneys would not submit a claim for my 2008 fall and why they and Liberty
Mutual are preventing me from getting to the bottom of what has caused all my pain. What type of corruption are they
covering up?

I think all the doctors were thinking that I was trying to put in a false claim because the X-rays of my neck, and my back did not show anything that was wrong! But they didnt think of my right side pelvis or my side hip and numbness in my right leg and right arm or they knew about it and were trying to cover it all up!

Ill tell you something, I Dominick, never told a lie in all my life. Because if you tell a lie, you have to live a lie and I dont live like that.
