Oct.&nov. 2011 unauthorized two $29.95 charges my account # came from?

They charged my credit card two different time, for the amount of $29.95 each time At the present time I don't know where they got my card information.

I called them and a form is being sent to me to fill out and after review I might get a credit. I wont hold my breath.
