Has another person attached to my report raising my rates

I am renewing my auto insurance and my rates have increased $100.00 more than the last 6 month policy at several different insurance agencies that I have checked with due to a female person by the name of WEBBER that is attached to my consumer report.

I have absolutely no knowledge of this person's existence and has never been attached to me in any way.

Can you please advise me as to what to do to eliminate this error.
