Sent me a bill in the form of a warning notice

Received a warning notice wanting me to mail them money for cancled insurance. The mailing address is CCS, PO Box 9134, Needham Heights, MA 02494-9134 which does not even match their main address which is C.C.S. Check Processing Center - 27, PO Box 55126, Boston, MA 02205-5126. This is a pretty good scam citing federal law, etc. I want to know how they got my information. I have turned this over to the Police Investigative Unit and DA Office for Tulsa, Tulsa County, Oklahoma. This is TOO MUCH!
