Dishonest, charging for items on policy they never provided

I have been with Safeco for a year now and just recently found out that I was charged an extra $99 a month because they are claiming they have sent me a form to sign stating that I opted out of uninsured motorist item on the policy which I did. I never received a form and the agent never said anything about it either. I would have asked them to fax the form. I checked with the packege as well they have sent me and sure enough there is no such form.

A month ago my wife got into an accident with an uninsured motorist. We called Safeco and sure enough, I asked if there is anything they can do. I expected a negative answer since I do know I did denided that option since it was quite a bit more. Now, knowing that I was charged for this option all along, I don't understand their way of business. I confrunted them with this information and obviously you can talk to a wall and get more respect rather then these dishonest company.
