Forces Homeowners into Default by Not Allowing to Make Payment

Every year for the last 5 years Bank of America has tried to force me into default if I come close to 3 months late by not accepting mortgage payments or putting payments into and escrow account and not applying towards loans. They tack on a $550 charge that no one can explain (surreptitious loan re-mod fee). This year they got more creative and sent charged me flood insurance retroactively, increased my monthly payment by 2 1/2 times and would not allow me to make payments since September unless I paid the inflated minimum payment which I could not afford. I used up my monthly cell phone plan over the last 3 months until I could finally get an answer as to why my monthly minimum payment increased to almost 3 times the amount. They would not allow me to pay online, by phone or in person unless I paid the inflated amount. Now they tell me I owe them almost $3000. My mortgage payment is is only a little over $200 per month.
I contacted Flood Zone Correction and found out what I already knew. My address was declared to NOT be in a Flood zone (by FEMA) since 2004.
In the meantime they have been sending me 2 Fed Ex Overnight letters for 3 months almost daily with paid return overnight Fed Ex Envelopes.
They try to throw me into a loan re modification every September and either wont accept payment or withhold it in escrow.
I am sure there are hundred of thousands like me and I don't understand why they have not been stopped form these egregious practices.
Today I got my maps and letter from Flood Zone Correction. They have not accepted a payment from me since September. Why isn't anyone doing anything about this? If they are doing this to me. They are doing it to many others.
