Stole $87.00 from my checking account

Prime Benefits took $87.00 from my checking account on September 20.

I checked my bank account online and saw there is a check issued to them. I opened the online image to see how the check does appear. The check did not have my signature and was not even mine.

What a ripoff! How Did they get my account information and why did my bank made a debit from my account when it doesnt carry my signature?
