Invalid Maintenance fee - customer service

Maintenance fee is deducted from my checking account at random when I have monthly salary deposits on to this account. Issue is pending since more than a year and still no resolution and help from the BOFA customer service. I would appreciate if they can refund me my money and correct my account setup to make sure invalid fees are not charged on my account in future.
