They withdrew $149.95 from my checking acct. Without my permission ripoff

On Jan. 4 I was going through my bank statements. I notice there was a withdraw from my acct. For $149.95. There was a 800 number so I tried to call it. It took me til Monday Jan. 8 to get a hold of them. I spoke to a lady named (Shautay) and asked her who gave they permission to withdraw the $149.95 from my acct. She said I had filled an application and I told her I didn't.
Since that day they had put the $149.95 back in my acct. On Jan. 16
