Ripoff by adding 5 point and not setting up my escrow account

I have very upset with Aegis Mortgage corporation.
I refinanced my mortgage with this company they charged me 5 point. I was promised that an escrow account was to be set up for my taxes and homeowners insurance that was never done.

My home was appraised at $93,000.00 I reduced the term to 15 years paid of a few bills was given an interest rate of 8.00%.

Which brought my loan amount up to $79,000.00
I recently tried to refinance and Had to have my home appraised again it was then appraised at $83,000.00
the loan was denied for insufficient collateral.
