Leave of absense

I had to take a leave of absense for the death of my mother. The agreement was i would be able to return with the same hours and at the same location. Upon me wanting to return my manager i.E. Tara speed told me to go put my application into one of the other salons that i could not have my job. She also sent in a false paper stating that i volunteered to quit. That was sent without my permission and no signature on the paper.
