Membership was not cancelled as promised and credit card was charged

I was a member of Gold's Gym in Tulsa, OK for about 6 months when I lost my job and had to cancel when I relocated back to Georgia. I went to the Membership Manager, Rick Doyle, and hand wrote a cancellation note.
