Customer Service Rep told me he took care of payment arrangement and never did, phone shut off

I called to make payment arrangements so that my service would not be shut off. I couldn't pay my bill until the following week. 3 times I called and they said I would have to call back because the computers were down. The 4th time that I called, after holding for 25 minutes, the rep who answered the phone said it would be fine for me to pay my bill the following week. I asked him if he needed my name or phone number to note the account, since he didn't ask me for anything, and he said no, he had it. Well my phone was shut off 4 days later. After going round and round with them, they agreed that if I paid a certain amount, they would restore my service. I had no service for 1 month, and they billed me for the time that I had no service, internet charges also. I have been trying since November to get a credit on my bill for the charges that I never incurred. No one will help me.
