Signed me up for Long Distance service without my permission

USBI signed me up for long distance service that I did not authorize. When I call them to ask how can they be registered as my LD company when I never signed up with them. They say they have my day of birth as prove that I did sign up with them, and that the date of birth is ALL they require to sign you up.
Beware! Request to listen to the recordings that they are obligated to have where you are supposed to be answering to a third party company confirming that you want in deed USBI's services.in my case they pulled up a recording of somebody that was not me accepting the service and giving my name and date of birth!
