Writing off the same credit account year after yea

I am writing you on the behalf of seeking justice and to inform you that Providian Bank has been reporting this credit card account xxxxx as a delinquent and/or charging-off every year since 5/1998. According to the documentation I received from 3 credit bureaus I noticed again that the same account was charge-off again in 3.

This account was closed by me back in 5/1996 after Providian quoted me one interest rate and bill me for a higher rate. Furthermore, dates have being changed; false information is being reported to the 3 bureaus from one year to the next by way of extending this item as R5/negative on my credit report. I have contacted them in writing for an verification of this debt to be provide to me in the form of an itemized account statement detailing explanation for this item however they have been non-responsive.

I absolutely dispute any claim that I defaulted in any obligation to Providian for any payment of any kind.
