Tried to rip me off victimized many consumers

I just recieved this form in the mail today and was gonna send off for it but decided to check the internet for any information on them. Luckily and unforunately someone else reported them i could not believe it was the same company i am sorry that it happened to him but i am very glad that it didnt happen to me. I cant see why this company is still sending out this things when they are already busted.
