Cashed my payment check but denies receiving it despite evidence from my bank

I obtained health insurance from Golden Rule through my professional organization (not from an employer; I am a consultant). I made a payment on June 23, making a mistake on the account number (a typo). Golden Rule cashed the check on July 3, but did not apply the funds to any account. Then they sent me a notice that my insurance was about to lapse because they did not receive the payment.
I called to ask the payment be applied appropriately. They came back saying they had no record of the payment.
My bank faxed a copy of the cashed check several times to two different fax numbers. Golden Rule claims never to have received any of the faxes.
My bank then sent a hard copy of the cashed check by mail. Golden Rule says it has not received the letter.
I have repeatedly called the customer service reps and have been told that someone is working on the case. At first they blew me off. After a three-way conference call initiated by my bank, they promised to have someone look into this within "24-48 hours." I have never received a call back. When I call to check on the matter, I am told to wait for a call from the mysterious specialist. I can never talk to a supervisor nor to this "specialist" who is working this issue.
The policy has lapsed by now.
