Office Max, Nettie Fuentes refused to send me my rebate after sending the required information to them over 5 times

Office Max and Nettie Fuentes refused to honor their rebate even after I sent them the correct forms over 5 times. The Attorney General even investigated them for not honoring rebates. They refused again after recieving the proper forms from them. They owe me $15. I contacted them over 20 times, and I recieved the run around each time. They finally started deleting my emails and stopped recieving my phone calls that they requested. They had no intentions of honoring their rebates.
