Unauthorized Charges on my Phone bill

Copy of email sent to Integravoice:

On our telephone bill of 11/13/07 we had a charge of $6.95 for "Integravoice" service. Not knowing what this was I called and was told that someone had ordered the service but I could cancel it and receive a credit on the next phone bill. I could get no information as to who actually signed us up for the service—I know that neither my wife nor I had signed up for it.

Anyway, the 12/13/07 phone bill arrived and instead of receiving a credit for the $6.95 as promised we got another charge for $6.95.

I called today and was promised that the service had been canceled and our account would be credited.

I just want to be sure that we receive no more charges from your company and that we receive credits for all the charges, plus late fees, to date.
