Sangrias Credit Card Fraud at Bar, Added Extra Money to Tab/Tip

My tab was for $10.00 and I left a cash tip, so the total was $10.00, which I signed off on. I got home and 2 days later, this place, Exit 66, d/b/a / Sangrias or Exit 66 Nightclub, charged me $39.00. I contacted the manager Juan, who offered NO help and NO return call. I called him back 3 times and he always put me in voicemail. He said on the one occasion we did speak that " it happens." NO, it does not and should not! I have bartended for years and never added extra tip or amounts after the tab was SIGNED. Beware of credit card fraud here - It has happened to my co-worker and cousin as well. They will be reported to my bank next!
