Update! The refund form you fill out and return or fax is going to a (mailbox etc.) type of place. The 8605 Santa Monica Blvd. Is a place called Earth Class Mail. Not Premier Membership Clubs. I called and "Haley" answered with a rather heavy far eastern accent. I went around and around about not authorizing a withdrawal of my account. She said to complete the form and return it to them at the given address. When I told her the Phone # didn't match the address she got very upset and rushed off the phone. Just a heads up everyone.
