Made an online purchase for my wife. At the end of the purchase got a "customer service questionairre" for possibly winning a $1,000 Wal-Mart Gift Card. One of the things they asked for was a cell phone number. A couple of days later I get a text from 1-703-3801784 announcing I had won the card and needed to fill out a form. I did (dumb) they got a lot of info, then form locks up and crashes.

Need to put a fraud watch on my name/accounts.
