Fraudulent Check Processed

I received notification from my bank Chase that a 99.95 charge had been processed through my checking. I was told that the check indicated it was authorized by me. The bank even went further and said that the date and time stamp were May 16 at 7:01am. On that date and time i was in fact a patient at Cleveland Clinic having a procedure done. When i called the company i was treated poorly, the woman identified herself as a supervisor and would not let me speak and she insisted that i had in fact authorized this debit from my account.
