They have been deducting $11.99 each month for about a year now from my debit card and I don't know how to make it stop. They are a bogus company. I don't know for sure how they got my info

This company has been taking money from my debit card for the past year. I did not sign up for this. I am not sure how they got my information.
I want this to stop, but I am not sure how to get it done. I have not used the card they sent to me.
They are basically stealing from me, and I want it to STOP... This is a bogus company with a lot of negative feedback. Please HELP...
