Dear Sir or Madam:
I am writing to demand the immediate charge back reversal on a fraudulent billing and
authorized withholding in the amount of $1200 made to my account 12/9/08. The amount is inaccurate. Cancelled three days prior to any alleged surgery date
1: Law requires full blood works of all patients medical history be preformed prior to any surgery bookings. This requirement is necessary in order to protect the safety of all patients and determine if a patient is a suitable candidate for liposuction procedure "Dr failed to obtain my blood work and forced surgery date without blood work or my authorization. My three days rights by law to cancel were fully executed. Kathy the facility manager was fully informed of cancellation and requesl
for the return of charges on the following dates 12/12/08,12/22/08,1/8/09 & 6/4/09 as
documented on recorded phone conversation. The front desk secretary was also fully informed via voice mail and personal request of the return of funds "which she nicely stated she forward to Kathy the above named. I am requesting that this error be corrected, That any finance and other charges related to the disputed amount be credited as well, and that I receive an accurate statement DETAILING THIS CHARGE BACK CREDIT
