Survey code - survey code to get $10.00 gift card

Today I purchased an outgoing order at your location on sepulveda and ventura blvd. I have received on my receipt that if I give my survey code number I will receive a $10.00 gift card. Well it has been near impossible. Each time I try it says if I fill out all questions I will receive $50.00. This on line is not allowing me to even find the place to insert the code. Is this really an honest offer to get the gift card.
