To authorize the check which would allow me to receive $57,000 from either PCH, Readers Digest, etc. I was supposed to call 1-514-638-5610. The contact name was Jason Wittoff. The check was drawn on Great Lakes Bank in Blue Island, Il. The routing number is 020XXX, and the Acct # is 071911XXX. The check had an official looking watermark on the reverse side. I file this report due to a prior report filed.
