For the second month in a row this company has charged me $19.95 for something I have no idea what. They have a toll free number on my account but it won't let you use it, Florida

This galaxybenefits has charged my checking account $19.95 for 2 months in a row. The bank has sent it back as unauthorized ACH and this costs $20.00 each time. I don't know who they are or how they got my checking account number. The phone number on my bank statement is 800-974-7813. When you call that number, it says it is a non working number. I'm tired of this and don't know what to do. Thanks
