Used my name for purchases unlawfully

To Whom it may Concern,

My name is Charlette Grove. I recently received a payment request from OSI Recovery Solution, Inc. It seems that someone used my name to order something from Columbia House. I have tried to reach OSI without any luck. There are no humans anymore running the phones.

I suspect that an X-tenant of mine used my name to order from your company. That persons name is Wayne Sanfacon. I do not have any more information to give you. I had to evict him for not paying rent.

I would like to have this account shut down and I am not going to pay for something I did not order.

The only information I have concerning account numbers and so forth are the numbers that OSI assigned to this account.
