Second Account Opened at Friend's Address Led to $692 Unpaid Bill

While giving dish network@ a friend as a customer, which I had ask about a new customer signing bonus. Their customer service rep. Assigned me a second acc. At my friends new add. Without my knowledge. Dish network@ or their executive resolution team is holding me responsible for a unpaid bill of $692. An has already put this on my credit report, all three bureaus. And will not take responsibility for this themselives.
