Deducted monthly premiums while claiming I wasnt covered under a dental policy. Asked for a refund and they refuse to refund payments

I am submitting this as required by Ms. Kari Paige as the next step of appeals. Here is my understanding of the deal.in Jan '04 my father Miguel * applied to have me on a family policy. He was paying the premiums.
Later we were informed because of age limitations this was not possible. So to have coverage we agreed on that the premiums would be taken out his acct and I would reimburse him for them. As I recalled we called 3 times in 2003.

First call was clarification that I was not able to be one their policy, the next for emergency coverage info and lastly because my dad noticed his being billed. I say we as I had the call on speakerphone so my mom and dad could be on the conversation and know what was being discussed. Plus being my dad was the policyholder the woman wanted him on the phone.

I remember calling to find if I was covered and what my policy number was.

I remember we gave him my dad's policy number and said mine was as I had to have emergency treatment. When I called I was told I was not covered and Blue Cross could not find me in their system. So I went to San Diego State University's Health Clinic and got a recommendation list of places to go for free emergency treatment for people with no insurance coverage treatment. I went.

Later my dad said he noticed that premiums were still being taken out of his acct, which I thought was odd being I was told again when inquiring on this that I still was not covered.

A Premium of $17.50 was still being charged to his acct. Finally in March we finally got a female on the phone who did some research as to why we were being billed if I was always told, I am not in their system and they could not find my name.

I finally was able to get that straightened up and received a bill for my coverage.
For the hassle and a half of not being able to use the policy being we were informed I was not covered until March 04, (those 13 months were paid) we are requesting of a refund of 12 months of premiums $210. Without interest.

I have since which paid my premiums.
We are looking forward to having this cleared up by 15 June.
