Charged credit card without knowledge, approval or signature

Arlington Family Dentistry office manager issues a treatment form to the patient after first exam and asks patient to initial the spaces with teeth numbers on it.

Patient is not advised that they intend to perform these services which patient does not approve Patient has requested one implant, recement one crown and teeth cleaning and that is all.

Dentist does not listen to patient, he extracts two gold crowns, and makes preparation to seat a crown on one tooth that had been filled by her own family dentist 3 months prior, Dr La did not listen. He made a crown for this tooth even though patient told him no! Now he wants to charge for this. This is fraud and unnecessary.
I originally contacted Dr. La since my dentist did not perform or install implants and my dentist gave me 2 sets of Xrays to take with me so that I would not be charged again.

I am so sorry that I contacted Dr La. I now have two missing teeth, a crown that was not necessary, charges for Xrays I did not need and a charge to my credit card for the crown which I have disputed.

The office manager advised me she would credit the crown charges. She has not done this. I paid for the other svcs even though I said no!

Instead they charged my credit card without my knowledge, approval or signature.

Instead they have charged my credit card in the amount of $1,896.00. I am disputing this charge.

Care Credit has been advised of this action and has refused to contact me.instead they have added a late fee to my account.

I am getting no where and don't know what to do.
I consider this fraud on their part.
