Did bad jobs, on a request to fix it became very rude and agressive, kicked me out from the office, dishonest billing

Dr. Christina R. Schemeler did a very bad job with matching the color for the front tooth filling and a bad measurement and a color pick for the tooth clip. When I asked her to fix that she didn't refuse to fix a problem with the clip but didn't want to redo the filling. Plus she became aggressive and rude.

Finally I was told that I have to go to a different dentist and she will give me a full refund for the clip. When I came to the office to take the records I was told that I have to pay for the clip because it was me who refused to fix that problem. And in her statement she wrote that i "flicked" my flip out at her. What I never did. Yes I was very upset with her job but I even didn't talk to her an inappropriate tone. I didn't know what to do so I called my insurance company and I was told that I was eligible for the clip and that insurance would pay for it. But I was told by her office manager that $250 out of $600 is paid by my insurance. So they lied to me.
