Store Card Opened in My Name Discovered After Care Credit Signup

I signed up for a Care Credit account with my vet. 2 months ago and just recently I got a letter from a fraud department from a store in NC. Letting me know that someone opened a store card with my name and all my info. Has this happened to anyone else?
