Rebate requires UPC code from phone box already recycled

My daughter has been a Verizon customer for three years, and on a recent trip to LA we stopped into a Verizon store and upgraded her phone. Great news - no cost for the phone as long as we renewed our Verizon contract for two years. We were given the paperwork for the rebate and told we had to fill it in and mail it, and then returned home the following day. Except that when I sat down to send in the paperwork, it turned out you needed the UPC code from the phone box, now long since recycled in LA. Since the UPC code is clearly printed on the rebate form, along with all the product and payment information, the only possible purpose of this requirement is to bank on the fact that many of us will have discarded the box before we read the instructions.
Verizon's rebate phone line answer: sorry, that's the deal! (And yes, they admit that many customers are in the same predicament)
