They never the promised rebate of $25 for the radios

My husband bought a FRS Radio 2 Pack from Frys. The product was from Columbia Telecommunications and it was a full amount rebate the amount was $25 which was never sent. It is almost over 3 months now. I still have a copy of my purchase rebate form and the upc code.
