Took money out of my checking account without my permission on 2 seperate occassions 9500 W Dodge Rd Omaha Nebraska 68114

I found out on my bank statement that Pass Port To Fun -My Offers was taking $16.95 out this month and last month. I never authorized any of this, and never even heard of the company's name. I would like to have a refund for the full amount that they have taken from me because I am on disability and do not have the money to spare for them to take out of my account. I think that they are very calculating and someone should do something to take care of this very soon.
