Refusal to pay for services

I provided training for this school in June and was told I would be paid the day of service. No Check. I invoiced the school and 1 month went by, I called the school and had to leave a message. Within the week I received a letter telling me I had to fill out W-4 forms, send in a copy of my driver's license and social security card in order to get paid.

I immediately called and had to leave a message again, explaining I am an independent contractor and I pay my own expenses, taxes, etc. Finally I received a phone call from Sheryl and she said, " this is the way this school pays anyone who does not have a Corporation, LLC, etc., After a heated conversation I decided I would fill out the information and get paid and be done with it.

Now almost 3 months after providing the service and 5 weeks after sending the necessary information I still have not been paid. I have called twice in the last week and no return phone call explaining why they are not paying me.
