Unauthorized deduction

I requested an effort teethwhitening collection and approved the cost of 4.99 plus delivery and handling just I had been billed $88.97 on 11/182008 for another package that used to do not approve the bundle was delivered to sender.in would really like credit for that and want to stop any more deliveries/costs. Thanks
