Take $35.00 a week from my pay check and sent a letter to my Dr. Saying They dont know who I am

I've got to shair this. They take about $35.00 a week from me. And pay my chropractor $80.00 a month. At least untill last April. My Dr. Has not seen a check as of last April 04. And here is the letter they have been sending him.
