Great West Retirement Services refuses to cash in my IRA. There customer service is giving me the run around and ignores my request to cancel there services

Back in 2004 the company I worked for Evergreen enrolled all of there workers at Great West Retirement Services. It is plan number 395001-11 on one paper or 395001-12 on another paper as there are many addresses this company claims. Back in April we have been trying to cancel our IRA yet customer services always has a run around answer to keep us from doing so. On August 23 from a PO box 173920 address in Denver, we received another statement which now has my birthday wrong and I was unable to enter the site access the web ste www.gwrs.com. The customer services is horrible and I doubt all ever get my $1500 back.
