Franklin Report Cancelled in Dec of Unauthorized transaction in Feb

On Sept 29 I bought a book online for $4.95. There was a trial offer that I could cancel anytime. On Dec 13, I received a phone call from http://www.newsmax.com / telling me that my card was about to be drafted and if I wanted to cancel, that I had to go online. I went online and saved the pages that confirmed that I cancelled. The ran my card on Feb 9 for $99.00 after I cancelled in December.
This is a unauthorized transaction. I need to cancel my card, and file a fraud report on my personal account.
